Job description
This Vulnerable Patients Care Co‑ordinator role with Viaduct Care, based in Stockport (SK1), places you at the heart of local primary care supporting people with frailty, dementia, learning disabilities and other vulnerabilities. You will work across the Stockport East and South PCN, carrying out home visits and clinic reviews, running EMIS searches, arranging and coordinating personalised care plans with patients, families and the wider health and social care team, and supporting NHS Health Checks and basic observations where required. The post is permanent, full‑time (pro rata on a 37.5‑hour week) at Agenda for Change Band 4 (£27,485–£30,162 a year pro rata). Essential requirements include GCSEs (grade C or equivalent) in English and Maths, PCI Accredited Care Co‑ordinator training (or willingness to work towards it), a full clean driving licence and access to a car, and a DBS check; candidates should be confident working both independently and within multidisciplinary teams and be comfortable with administration, data collection and safeguarding responsibilities.
