In this fixed‑term, full‑time Band 2 role (12 months, £24,465pa) you’ll work as part of the Out of Hours Urgent Community Response Team to help avoid hospital admissions by enabling nurses to provide urgent care in patients’ homes, care homes, clinics and within the travelling community. Duties include driving and locating nurses to and from visits, supporting the nurse co‑ordinator with referral registration and visit allocation, using SystmOne and assistive technology for mobile working, and stepping in to assist with moving, handling and basic clinical tasks under nurse supervision. This role suits practical, empathetic people with NVQ2 in Health or Social Care (or equivalent), a minimum of one year’s driving and some health/care experience, GCSEs (or equivalent) in English and Maths, and a full, valid driving licence. You’ll join a supportive, team‑focused out‑of‑hours service where good communication, calm judgement and flexibility matter; training opportunities are available to broaden skills in areas such as palliative and long‑term condition care.
