As Band 4 Integrated Neighbourhood Team Coordinator based at Solihull Hospital (B91), you will be the central navigator linking a designated Primary Care Network with the INT multidisciplinary team. You’ll manage and coordinate patient information from primary care, community and mental health services and the voluntary sector, using the shared care record and the Case Management Tool to prepare cases for triage and MDT meetings, take clear minutes, record outcomes and follow‑up actions, and produce reports. The role supports development and review of standard operating procedures, requires strong administrative and IT skills, and carries responsibility for maintaining data protection and confidentiality. This permanent, full‑time post offers flexible working within Band 4 (Agenda for Change), salary £27,485–£30,162 a year.
