Job summary
The Integrated Neighbourhood Team (INT) Coordinator will be a central point of contact and navigator between their designated Primary Care Network (PCN) and the INT multi-disciplinary team (MDT) establishing effective communication processes across primary care, community services, mental health services and the voluntary/third sector.The INT Coordinator will work in collaboration with the INT core team and will be responsible for liaising with designated leads to manage the coordination of patient-related information to support the MDT's triage and MDT meetings including capturing outcomes and evaluation.To be an active participant of the INT MDT assisting with the decision-making process.
Main duties of the job
Patient/ Client Care*Incidental contactPolicy and Service Development*To support the development, implementation and review of standard operating procedures (SOPs) for INT level work and processesInformation Resources*To manage and coordinate the information flow between all MDT Team members and any other co-opted stakeholders, ensuring the Case Management Tool (CMT) records all citizen/patient-related information as received via the shared care record and other designated information systems.*Collect, collate, and manage citizen/patient-related information from clinical systems, partnering with other organisations as needed.*Provide support to INT members in carrying out holistic citizen/patient investigation through information collection, ensuring INT members are well prepared for MDT meetings.*Take clear minutes of discussions at triage and MDT meetings.*Record, document, and communicate clear actions and outcomes from Triage meetings, MDT meetings, intervention conversation and follow-ups using the CMT.*Use information systems to extract data to populate reports as required*To establish and maintain systems for the management and storage of information adhering to Trust policies and procedures, particularly in relation to confidentiality, health and safety, equal opportunities, and data protection.
