The Mid Dementia and Frailty Pathway provides a comprehensive assessment, diagnosis and treatment supporting patients of any age with dementia as well as older adults, who have functional mental health needs with an associated age-related frailty. You will work as part of the multi-disciplinary team supporting patients with SMI / a secondary care mental health need/ dementia and frailty. in addition the post holder will support the clinical manager with some admin tasks therefore will need to be literate in IT/ computer skills. The role is Monday-Friday 5 days a week 9-5 and it is essential that you can drive and have access to a car, as you will be working across Mid Essex You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services. Please note this is a community position, and it is essential criteria for the applicant to have a valid UK driving licence and able to drive, and to have access to a car for work purposes. The post holder will work as part of the multidisciplinary team. The post holder will contribute to assessment of both physical and mental health needs. Focusing on the wellbeing of those with serious mental illness, and demonstrating an understanding of the relationship between physical and mental wellbeing To work with other agencies to deliver a systems approach in ensuring patients mental and physical health wellbeing needs are met. The post holder will undertake annual physical health checks of patients who have been identified as having severe mental illness. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests. The post holder will arrange physical health assessments with the service user both at home, in clinics, and at GP surgeries; taking a proactive approach to engaging patients with SMI in the importance of annual physical health reviews. To provide information on health promotion including healthy eating, sport and exercise and smoking cessation To work with patients and relevant staff to develop care plans and complete the electronic physical health assessment form on electronic health record; ensuring the information is shared with patients and GPs.
