Step into a hands-on Care Coordinator role within Harness North PCN, supporting 10 GP practices across NW10 (Park Royal / Central Middlesex). There are three positions available. This full-time, fixed-term post (16 months) pays £28,275 and focuses on co-ordinating personalised care for vulnerable and complex patients. Day-to-day you will liaise with patients, carers, GPs, the local authority and community partners; manage referrals and appointments; assist with assessments, care plans and follow-ups; signpost to local resources; and use data to identify trends, improve efficiency and reduce health inequalities. The role requires confident use of EMIS (searches, templates and reporting) and familiarity with digital tools such as Abtrace, iPlato, Accurx, Excel, Teams and SharePoint to support smooth practice workflows and better patient outcomes.

This role is ideal for carers and experienced administrative or project staff working in healthcare who want to move into care coordination and service improvement. You should have experience of healthcare systems, good communication and problem-solving skills, GCSEs (or equivalent) in English and Maths, and competence with Office applications; a DBS check is required and Personalised Care Institute training is desirable.
