An exciting opportunity is available with Urgent Community Response North .We are looking for an enthusiastic individual to join the team and undertake a newly created Care Coordinator role - completing safe and efficient allocation of urgent patient visits.

To co-ordinate patient appointments for the North Urgent Community Response service - following set processes, onto ledgers, whilst considering relevant data including geography, clinical skill mix and staffing levels to ensure patients are seen by the most suitable staff in a urgent timeframe. Scrutiny of appointment
