Make a real difference to patients at Rock Court Surgery as a Care Navigator based in Old Swan, Liverpool (L13). This hands-on role puts you at the heart of the practice: you will manage the appointment system, triage and care‑navigate at the front desk, over the phone and online, signpost patients to the right services, and support clinical staff with admin tasks such as SARs, DVLA forms and data entry. The role is permanent, 30–32 hours per week (Monday–Friday, typically 12:30–18:30 with Thursdays to 20:00), offers both full- and part-time flexibility, and includes on‑the‑job training — GP experience or EMIS knowledge is desirable but not essential. A DBS check is required and the current pay rate is £12.21 per hour (subject to change).

You’ll join a welcoming team of GPs, nurses and seven care navigators within a practice list of around 5,600 patients, and be supported by wider PCN services such as pharmacists, social prescribers and paramedics.
