Support timely, safe discharges as a Case Management Assistant at Royal Surrey NHS Foundation Trust in Guildford (GU2). This permanent, full‑time Band 3 role (salary £26,240–£27,928 pa, pro rata, includes HCAS) sits within the Case Management and Discharge Hub and rotates between ward‑based and office duties. You’ll work alongside multidisciplinary teams and Case Managers to complete referral and discharge paperwork, book transport, manage onward referrals, resolve internal delays and help reduce unnecessary bed days and readmissions. The post suits someone with prior healthcare or administrative experience, an NVQ Level 3/BTEC in Health and Social Care (or equivalent), an IT qualification such as ECDL, confidence using clinical systems (e.g. Cerner desirable) and willingness to undertake a DBS check.
