Main duties of the job
The focus of the Case Management Service is to coordinate seamless patient flow through and from the hospital. The Case Management Service coordinates all patient's pathway through the hospital ensuring patients are transferred to the most appropriate care setting in a timely manner whilst providing a safe, effective and positive patient

Job responsibilities
The Case Management Assistants will provide a comprehensive and accurate administrative and patient related support function to the Case Management Service (part of the wider integrated care team).The post holders will rotate through both the ward based and office based functions.Case management assistants on the wards will receive day to day direction from Case Managers to support timely discharge.The case management assistant in the Discharge Hub will receive day-to-day direction from the Case Management Team Lead. This will include prioritising workload and completing tasks within agreed timescales.Both functions will work together to provide a seamless service for patients, families and internal and external partners with the aim to facilitate timely transfer to an appropriate care setting and reduce readmissions.
