An exciting opportunity has arisen for a full time Band 6 Community Mental Health Nurse for 30 hours per week within the Complex Intervention and Treatment Team (CITT) in North Somerset. We are an integrated health and social care team that provides support for later life service users across the locality with both functional illness and dementia.
You should be an organised, highly motivated and enthusiastic individual who has excellent communication skills, a flexible approach to work and be committed to working with later life service users. Previous experience of working with this client group would be an advantage. We are a very supportive team offering a range of opportunities for supervision.
We would welcome anyone who would like to come along for an informal visit.
You will be a key worker and will co produce Safety Assessments and Personal Wellbeing Plans with your service users and their families/carers.
This will include signposting toand navigatingcommunity resources
You will be completing Care Act assessments, if indicated, using the local authority IT system LAS.
We operate a duty system, staffed by 1 qualified person each day.

Most challenging part of this role
To support the care of individuals within a defined caseload, to develop increasing skill and confidence with a wide range of health, social care and wellbeing needs, setting priorities and working in community based settings. To contribute to the development of a culture where the personal and positive aspirations of service users are the primary focus of the plan, while developing and maintaining individual skills in delivering a range of therapeutic interventions. This must be achieved whilst respecting the dimensions of personal choice, diversity and culture and the benefits which these bring to the therapeutic relationship.
Policies and Procedures
Trust employees are expected to follow Trust policies, procedures and guidance as well as professional standards and guidelines. Copies of Trust policies can be accessed via the staff intranet (Ourspace) or from your manager.
In particular, attention is drawn to the Trusts arrangements in relation to safeguarding children and vulnerable adults as well as infection prevention and control.
All employees are expected to be familiar with the Trusts approach to risk management, take a risk management approach to their own work and take responsibility for the management of the risks they own.
Confidentiality
Much of the work is of a confidential nature. This means that no discussion should take place about the care, needs, or activities of any service user, except in the clear interest of that service user or other members of staff. Staff are reminded that personal information concerning colleagues is also confidential.
