As part of the pioneering Community Frailty Virtual Ward in Plymouth, South Hams, and West Devon, you will deliver both proactive and reactive care to our most vulnerable. This full-time position offers a dynamic blend of patient interaction and clinical challenge, with the chance to oversee a caseload, engage in delicate advance care planning discussions, and contribute to the broader goals of Comprehensive Geriatric Assessment and collaborative care planning.
