- To provide a holistic patient assessment and structured carer interview.
- Responsible for completing specialist initial assessment and interpretation of results to provide a baseline for future treatment/care.
- To work autonomously and show initiative when planning clients care.
- To deliver diagnosis and offer expert advice regarding the diagnosis and treatment and provide education/support to both clients and carers.
- To provide education on the different kinds of dementia and how to manage the symptoms of dementia.
- To monitor and evaluate patient response to treatment, with autonomy to recommend and implement changes as required.
- To work as a supplementary/independent prescriber according to Trust policies.
- Autonomy to accept/re-refer/reject referrals to the service as appropriate.
- Autonomy to refer to other professionals/agencies as required.
- Required to visit clients and carers in their own home environment to complete initial assessments, reviews and treatment around care plans

The role will involve, but not be limited to you performing the above.
What you'll need:
- A minimum of 6 months paid experience in a Community Mental Health hospital environment within the last 3 years
- Comply with NMC Professional Registration and keep up to date with current nursing practices
