Coventry and Rugby GP Alliance is seeking an Admiral Nurse to provide person-centred and relationship-centred dementia care in a flexible hybrid working environment. The role focuses on supporting families affected by dementia through expert guidance, specialized assessments, and practical solutions. Key responsibilities include:

  • Conducting bio-psychosocial assessments and developing individualized care plans.
  • Providing emotional support and psychosocial interventions to enhance the well-being of individuals with dementia and their families.
  • Educating and training staff on best practices in dementia care.
  • Collaborating with health, social, and voluntary services to ensure coordinated care and aid in care transitions.
  • Advocating for the rights and needs of people with dementia and their carers, while ensuring compliance with relevant legal frameworks.

Additionally, the Admiral Nurse will engage in clinical supervision, professional development, and contribute to the continuous improvement of dementia care practices through evaluation and audit initiatives.
