Band 6: £37,338 to £44,962 a year per annum/pro rata  PERMANENT  GOOD SALARY 

We are currently seeking a Community Sister to join the Crook Community Nursing Team. The successful applicant will work across the Weardale locality and the surrounding areas from an office base in Crook. The team cover 8am until 9pm 7 days a week on a duty roster.The post holder will be a caseload holder to ensure the provision of clinical management to District Nursing and Intermediate Care Services. The role involves providing holistic nursing care to adult patients in their own home or residential care setting. Successful applicants will be required to hold a first level nursing registration with the NMC, additional training specific to the role can be provided.

To support coordination and delivery of high quality nursing care that, where possible, meets identified patient need, supports the aims and objectives of the integrated health and social care team, and remains within the boundaries of available resource. Predominantly complex clinical care and caseload management with some coordination and supervisory responsibilities for health care staff between Bands 2 to Band 5. The Community Nursing Team provide both planned and reactive care including Urgent Community Response.

About us

The sucessful candidate will provide a variety of nursing care to patients; including wound care, administration of medication, palliative care, management of bowel and catheter care. The role involves assessment of patient care needs, care planning, delivery of care and review. The sucessful candidate will be required to work independently as part of the wider Nursing team reporting back to the Team Lead. To support patient safety and clinical effectiveness providing expert evidence based clinical advice and support staff within the team.
