City Health Care Partnership CIC is seeking three Health Care Assistants (Band 2) to join their Intermediate Care Team at the East Riding Community Bedded Unit within East Riding Community Hospital. This role involves providing care to patients requiring rehabilitation, including those with complex health needs, stroke rehabilitation, and end-of-life care, either in a community bedded unit or at home. The unit operates 24/7, offering a range of shifts, and the post holders will be responsible for delivering personal care and supporting the rehabilitation process to promote patient independence and safe discharge into the community.

We have an exciting opportunity for a 3 Health Care Assistants (Band 2) to join the Intermediate Care Team based at East Riding Community Bedded Unit within East Riding Community Hospital. The Intermediate Care service operates across Hull and East Riding providing care for patients requiring rehabilitation either in a community bedded unit or own home.

Working in an active therapy and rehabilitation unit will give you the opportunity to look after patient requiring level 4 rehabilitation, stroke rehabilitation and also patients receiving end of life care.

The unit is covered 24 hours a day, 7 days a week. We provide a range of shift such early, late, long day and night shifts.

The post holder will work as a band 2 Health Care Assistants in the East Riding community hospital Community Bed service.

The post holder will be responsible for providing personal care and supporting the rehabilitation of people with complex health needs to enable independence and a safe discharge into the community.
