The Cornwall Partnership NHS Foundation Trust is looking for dedicated professionals to fill the roles of Band 5 Primary Care Dementia Practitioners and Band 6 Memory Assessment Nurses within its Dementia and Older People's Mental Health Team in Bodmin, St Austell, and Falmouth. These community-based positions are flexible, allowing for a balance between personal and professional schedules while making a meaningful impact on the lives of individuals living with dementia.

Memory Assessment Nurses will focus on delivering comprehensive memory assessments and supporting patients and families through the diagnostic process, while Primary Care Dementia Practitioners will serve as vital links between primary care and mental health services, offering ongoing support and education for caregivers. Both roles involve managing community caseloads and require successful candidates to demonstrate their ability to meet travel demands for visiting various locations across the county.
