DEMENTIA EMPATHY MENTAL HEALTH MULTI-DISCIPLINARY NHS VENEPUNCTURE
The Sussex Partnership NHS Foundation Trust is seeking a full-time Community Health Care Support Worker based in Chichester, specifically within the Dementia and Older Person's Mental Health Team. This role entails supporting individuals living with Dementia and severe mental health issues, working closely with community nurses to deliver high-quality care. The Community Support Worker will engage in clinical visits, assist in the development and evaluation of care plans, and ensure patient needs are prioritized. Candidates should possess strong communication skills, empathy, and a commitment to promoting social inclusion and independence for service users.

The position requires the ability to travel within work hours, work as part of a multi-disciplinary team, and effectively address any patient concerns or risks. Applicants should have NVQ Level 3 training and a basic understanding of health procedures and mental health legislation. The work schedule is Monday to Friday from 9 am to 5 pm, and vaccination against COVID-19 is encouraged for the safety of all involved.
