The Northern Care Alliance NHS Foundation Trust in Salford is seeking both experienced and newly qualified Social Work Practitioners to join its Integrated Care Teams. The role involves delivering community-led support, promoting independence through reablement, safeguarding adults, and ensuring high standards in assessment and person-centered support planning. Practitioners will operate with considerable autonomy, manage complex caseloads, and will be expected to have in-depth knowledge in specific practice areas.
