Greater Manchester Mental Health NHS Foundation Trust is seeking a qualified Social Worker/Care Coordinator to join the North Manchester Later Life Community Mental Health Team. The role involves working within a multi-disciplinary team to support service users through commissioning and care purchasing tasks, while adhering to the Mental Capacity Act 2005. The successful candidate will conduct assessments for clients and their carers, provide care coordination, and participate in initial assessments for new referrals as part of the team’s duty roster.
