At Lister House Surgery in Derby, the role of a Clinical Pharmacist offers an engaging opportunity to enhance patient care by leveraging your expert knowledge in medicines management. As a key part of our dynamic multi-disciplinary team, you'll conduct structured medication reviews, guiding patients in achieving optimal outcomes from their medications while addressing health inequalities. You’ll support a holistic, patient-centred care approach, manage frailty and care home patients, and lead on initiatives like the Stop Over Medication Programme (STOMP). Your expertise will be pivotal in offering specialist support, reducing medication waste, and spearheading quality improvement and safety protocols, including deprescribing and drug monitoring processes.
