Engage in high-quality, patient-centred healthcare, overseeing vital medicines-related care for a diverse community of over 115,000 patients across ten GP practices in North Leeds. Your responsibilities will include reviewing medicines post-hospital discharge, reconciling clinic letters, and handling medication queries with professionalism.
