Band 7: £54,320 to £60,981 a year per annum inc HCAS (pro-rata)  FIXEDTERM 

  • To provide a service across St George's Hospital to Teenage and Young Adults with cancer with links to the Royal Marsden Hospital in Sutton.
  • The Clinical Nurse specialist will act as a key worker to patients and their families, helping to coordinate the care of TYAC patients being treated at St George's including providing specialist advice, care and support to patients, families and where appropriate clinical teams.
  • Provide specialist advice and support to patients and carers in terms of disease, treatment information, symptom control, palliative care and bereavement support.
  • To act as an expert resource in the care of TYAC patients to colleagues at St George's Hospital.
  • Demonstrate nursing and professional leadership with the Teenage and Young Adult Multidisciplinary Team, initiating, leading on and supporting service development projects.

  • To work alongside site specific clinical teams to provide high quality care and support to Teenage and Young Adult Cancer (TYAC) patients diagnosed with cancer at St George's Hospital.
  • To work closely with the TYAC and clinical teams at the Regional TYA Principal Treatment Centre.
  • Provide emotional, psychosocial and clinical support from the point of diagnosis, during and after treatment.
  • Advise patients and supporters regarding any aspect of treatment plan, side effects, symptom control, discharge planning and palliative care in collaboration with site specific clinical teams.
  • Where appropriate ensure new patients are assigned a Social Worker and liaise closely with all members of the MDT throughout the cancer trajectory, taking an active role in contributing to the multi-disciplinary meetings.
  • Liaise closely with schools/colleges/universities/employers as appropriate, acting as a resource in TYA cancer care and providing a supportive role as required.
  • Ensure that patients are being offered a holistic needs assessment at thebeginning and end of their treatment, and at other key moments throughout their treatment.
  • To act as a key worker to Teenage and Young Adult patients.
  • To accept referrals and assess individual need, implement clinical practice and evaluate its effectiveness, referring appropriately to services in the acute and community setting.
