The Sussex Partnership NHS Foundation Trust is seeking Band 6 Occupational Therapists (OTs) and Registered Mental Health Nurses (RMNs) for the role of Intensive Support Practitioner in Eastbourne. This position is part of the development of Neighbourhood Mental Health Teams aimed at transforming community care for older adults with mental health needs. Practitioners will collaborate with a multi-disciplinary team to deliver assessments, treatment, and intensive support to enable individuals to receive care in their homes, thereby reducing the likelihood of hospital admissions.
