University Hospitals Dorset (UHD) is seeking an Advanced Clinical Practitioner (ACP) to join our forward-thinking Stroke and Neuro Service, working across our Stroke Unit and our Early Supported Discharge (ESD) Teams — a leading service that transforms patient recovery by delivering specialist care at home. We are able to create this new role due to recent investment in our service, and are excited to grow our team. Come and join us by the sea, at one of the biggest stroke service in the UK! Interview Date: 19th November 2025 Why the Stroke Service at UHD? • Transform patient outcomes:Join a service that is continually evolving how we deliver stroke and neuro rehabilitation. • Multidisciplinary collaboration:Work closely with physiotherapists, occupational therapists, speech and language therapists, nurses, psychological, therapy assistants and Stroke Association support workers, in a vibrant, collaborative setting. • Autonomy & impact:As an ACP, you will take a lead clinical role assessing, managing, and planning care—making crucial decisions in how we deliver care to patients and their families. You will join a friendly leadership team and will be involved in both research and service improvement. • Innovative models of care:Be at the forefront of delivering integrated, patient-centred care that reduces hospital stays, prevents readmissions, and accelerates recovery. UHD are investing in services across the Trust, with many being transformed and developed following merger and the New Hospital Programme. Please note if a role or service relocates as part of a planned move, excess mileage will not be reimbursed. Travel from home to the new work base will be classed as a normal commute. Any other changes will be managed under relevant Trust or national terms and conditions. The Candidate We’re Looking For: • A qualified ACP with a passion for neuro and stroke rehabilitation – and for implementing new and innovative ways of working • A strong clinical decision-making skills and a confident autonomous practitioner • An excellent communicator and team player, able to inspire and motivate patients and colleagues alike • Committed to continuous learning and service development • Energetic, adaptable, and proactive – ready to thrive across the dynamic inpatient and community settings The post holder will contribute to both the Stroke Unit and the Early Supported Discharge Team, with duties divided on a 60:40 basis, supporting seamless patient care across the stroke pathway.
