Support and shape local adult care as the Connecting Lives Coordinator for North Somerset Council in Weston-super-Mare. You will recruit, assess, approve and support self-employed Connecting Lives Carers and Support Workers, match vulnerable adults to safe, personalised placements and hold your own caseload under the direction of a qualified professional. The role covers adults with learning disabilities, physical disabilities, mental health needs, dementia and other vulnerabilities, and requires excellent record-keeping, multi-agency communication and confident use of safeguarding practice.
