Assist with all aspects of personal care (continence support, showering, bathing, etc.). Monitor and report on individuals physical and mental health and behaviour. Record and report relevant information to the Shift Leader. Act as a keyworker for named individuals. Encourage and support leisure, social activities, and holidays as per person-centred plans. Deliver person-centred care with respect, privacy, and dignity. Support and maintain relationships between individuals and their families, friends, and significant others.
