Rehabilitation Support Worker - Neighbourhood Rehabilitation Service Job details Posting date: 29 October 2025 Salary: £24,937.00 to £26,598.00 per year Additional salary information: £24937.00 - £26598.00 a year Hours: Full time Closing date: 12 November 2025 Location: Barnsley, S70 3RD Company: NHS Jobs Job type: Permanent Job reference: C9378-B2228 Apply for this job Summary JOB SUMMARY The post holder will: Under the direction of registered health care professionals/associate practitioner, perform delegated routine interventions and care tasks, for which they have received training, seeking support as and when necessary. As part of the neighbourhood rehabilitation team, support appointments with service users in their homes and the community (including care homes), primarily lone working under direction or with other team members to provide the highest standards of care and compassionate planned care to people that maximises their recovery and rehabilitation potential. Support service users with personal care tasks, including bathing, toileting (such as changing and emptying waste bags, e.g., catheter or stoma, and assisting users in emptying and cleaning their own bags), dressing, and administering pre-dispensed medication (using a MAR chart, including the application of creams and eye drops, under the tongue, or ocular medication). Prompt and supervise service users, to build independence in managing their medication. May provide additional assistance and demonstration in use of medication, documenting any difficulties for the service user. Looking at any wounds, dressing and pressure areas and reporting any concerns. Look for any signs of agitation or distress. Assist with meals and drinks, help patients with their appearance, and document all activities as part of standard clinical record-keeping procedures. Support service users follow prescribed therapy care plans to promote their rehabilitation, including bed exercises, stair practice and balance exercises. Undertake training and on the job learning to understand the purpose and effects of these exercises. Build confidence and independence with patients through increased mobility. Deliver care within well-established policies, procedures, protocols, guidelines and established care plans within own competence and skill set. Where appropriate, encouraging and motivating service users to develop daily living skills, undertake mobilisation exercises and follow a personalised treatment plan. Understanding where this may be affected by a long-term condition eg dementia. Apply knowledge within scope of role and familiarity with service users to notice a change or perceived deterioration in service user presentation etc. i.e. wounds, bed sores, mental health, support escalation / reporting of this to ensure the service user needs being met. Provide feedback / updates to the registered professional/ team to contribute to the care planning process. Understand the principles underlying care planningeg, enabling the service user to reach an optimum level of functioning/ independence/recovery. Assist therapists in supporting people to carry out everyday tasks. Support in the delivery of the service in performing routine administration or housekeeping tasks. To follow a care plan, advise and guide service users, family and carers in following prescribed interventions and make the appropriate adjustments within own competence and skill set and be able to communicate any changes to service users and registered health care professionals. Apply basic knowledge of longer-term health conditions, within the remit of care planning, providing advice, support and feedback to a registered professional. Exercise a degree of independence, initiative and judgement and consider different approaches when delivering tasks. Assess situations, eg following falls prevention guidance identify and resolve potential problems, within own competence. Undertake the physical aspects of care eg, moving and handling of service users. Use items of equipment in delivering care eg, hoists, wheelchairs etc. (as appropriate to the role), through the relevant training with peer support and experience, ensuring appropriate instruction on their use has been received. Support the service, through delivery of assessed basic equipment to service user homes, for example commodes, toilet seats, pressure cushions and walking frames. Demonstrating use of equipment and aids, fitting, adjusting and safety checks aids before use Demonstrate sensitivity, empathy and active listening skills always managing and maintaining a positive appropriate relationship with patients and their families / carers. Ensure the care experienced is positive and service user safety is maintained. Treat everyone, e.g., service users, their relatives, visitors, colleagues etc, with respect, dignity and courtesy and in accordance with the Trusts Values. Follow and ensure routine care is provided in accordance with well-established policies, procedures, protocols, guidelines and established care plans. Be required to concentrate for periods of time, depending upon the care intervention/care plan. That includes undertaking personal care, engaging with service users, documentation and raising issues of concern appropriately. Whilst lone working be able to respond to verbal / physical aggression or challenging behaviour (including risk to self); recognising and responding to changes in presentation, using conflict resolution, including escalating as required in order to maintain own personal safety and accessing support for service users. Respond appropriately to emergency situations. Ensure appropriate reporting, including incident reporting (Datix) completion. Participate in a flexible rostering arrangement providing 7-day care including unsocial hours (as appropriate to the role). Use a lone working device.
