Based at Epsom with Surrey Downs Health & Care and employed by Epsom and St Helier University Hospitals NHS Trust, this part‑time permanent Rehabilitation Support Worker post sits within the Recovery At Home team, supporting timely discharges and preventing hospital admission. The role (salary band £29,651–£31,312 per annum pro rata, including outer HCAS) delivers delegated clinical and rehabilitative care in people’s homes and community settings, working to care plans and under the supervision of registered practitioners. You will carry out observations and practical tasks such as vital signs, urine testing, blood glucose checks, venepuncture, ECGs, PEG/tube feeds, wound care and removal of clips/sutures, assist with medicines and personal care, use digital records (EMIS/ICM) and contribute concise handovers to the multidisciplinary team. The service runs seven days a week between 08:00–20:00 and requires flexible shift working across the week; there is scope to discuss patterns to help balance work and life commitments, and you may be asked to work at other Trust sites as needed.
