This rewarding role focuses on working with young people transitioning from Children’s Services to Adult Services, ensuring a smooth and person-centred journey into adulthood. Key Responsibilities: Support young adults with learning disabilities transitioning from children’s to adult social care services. Undertake person-centred assessments, create care and support plans, and coordinate services tailored to individual needs. Work collaboratively with health, education, and community partners to ensure seamless transitions. Promote independence, inclusion, and well-being for individuals and their families. Maintain accurate case records and ensure compliance with statutory responsibilities and local authority policies. Essential Requirements: Degree or Master’s in Social Work and current SWE registration. Previous experience working with adults with learning disabilities or transition cases.
