Be part of Guy’s and St Thomas’ NHS Foundation Trust as an Integrated Care Pharmacist within Integrated Local Services, delivering high-quality clinical pharmacy across Lambeth and Southwark. This full-time, permanent role (salary band £56,276–£63,176 p.a. inc HCA) involves providing patient-centred care for services such as GSTT @Home, Integrated Care Southwark and Integrated Care Lambeth. You will work in community settings — health centres, community bed-based units and patients’ homes — leading domiciliary medication reviews, coordinating and implementing pharmaceutical care plans, and ensuring medicines are prescribed, handled and administered safely. The post requires close, routine collaboration with community matrons, GPs, therapists, consultants, community pharmacists and adult social care to optimise medicines use and improve patient outcomes across multiple care pathways.
