As a Band 6 Specialist Clinical Lead Outreach Nurse with the Focus Homeless Community Mental Health Team at North London NHS Foundation Trust (recruited by Camden and Islington NHS Foundation Trust), you will deliver specialist mental health assessment, treatment and care planning for adults (18+) who are rough sleeping or within the Camden Adult Homeless Pathway.
