Support adults with learning disabilities in the community from a base in Warrington (WA5 1UD) as a part‑time, permanent Health Care Support Worker with Mersey Care NHS Foundation Trust. This role, salary £24,937–£26,598 pro rata (job ref 7573579/350‑SC7573579), involves putting care plans and programmes into action, recording and reporting outcomes, and acting as a nominated worker for individuals who may present extreme challenging behaviour, changes in mental health, forensic needs or complex physical health needs. You will help people develop self‑help, communication and social skills, promote independence with daily living, support access to other health services and assist with group activities and therapeutic programmes under the direction of qualified practitioners. The post includes monitoring mental and physical health, maintaining accurate records and contributing to service improvement and user/carer involvement; flexible working requests will be considered.
