Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. Home First Assessors work closely with people who use our services, their families, friends and carers, who all play an invaluable role in how our users experience our services. They are expected to be kind and responsive, professional and informative and contribute to the quality of the services provided across the Health and Social Care economy. Holistic assessment of ongoing care and support needs both personal activities of daily living and social needs. Empowering the person to have the confidence to maintain independence for as long as possible through risk assessment, care planning and sign posting. Creation of detailed, person centred care plans. Give advice and guidance to access equipment /minor adaptations for the home. Oversight and supervision of the Support Workers on the team. Please see JD for comprehensive breakdown.
