As Senior Practitioner / Community Health and Social Care Coordinator — Neighbourhood Team Lead you will manage Integrated Care Coordinators and Community Inclusion Workers, shape multi‑agency working across health, social care and the voluntary sector, and drive person‑centred practice that strengthens community resilience.
