Lead an integrated neighbourhood team from Oakham and play a central role in helping adults and their carers live safely, independently and connected to their communities. This permanent, full‑time role (37 hours) pays £44,075–£47,181 pa plus benefits, with hybrid working (up to two days remote) and a supportive approach to flexible, agile patterns of work. As Senior Practitioner / Community Health and Social Care Coordinator — Neighbourhood Team Lead you will manage Integrated Care Coordinators and Community Inclusion Workers, shape multi‑agency working across health, social care and the voluntary sector, and drive person‑centred practice that strengthens community resilience. The post is suited to experienced practitioners — Social Workers, Nurses, Occupational Therapists, Physiotherapists or those with professional qualifications such as DIPSW, BASW or CQSW — who want a leadership role that combines hands‑on casework with strategic development.

You’ll coordinate complex cases, provide specialist advice on safeguarding, oversee performance and budgets, and maintain accurate records (including reports and court documentation where needed). The right candidate will hold registration with Social Work England or the relevant professional body, bring proven experience in adult social care or health settings, have a track record of supervising staff, and possess confident assessment, decision‑making and IT skills.
