Assistant Practitioner – Mersey Care NHS Foundation Trust, Southport (PR8). Play a hands‑on role within ICRAS (Integrated Reablement and Assessment Service), supporting rapid responses that prevent hospital admissions and speed safe discharges. This Band 4, permanent full‑time post (£27,485–£30,162) sits across rehabilitation hubs, the care transfer hub and community services, working in a multidisciplinary team of advanced clinical practitioners, nurses, physiotherapists and occupational therapists. You’ll deliver therapeutic interventions, accept a manageable caseload, communicate with patients and carers from diverse backgrounds, lead health promotion work and act as a strong advocate for people rebuilding independence at home.
