Job summary
This is a one-year part time role within the Macmillan Personalised Care team working with patients who have been newly diagnosed. Ideal as a fixed term/secondment for people looking to work in the cancer support roles that are available.If you are applying as an internal applicant please note: 'As per the secondment policy, please ensure you have your line manager's approval to undertake the secondment prior to interview'.
Main duties of the job
The post holder will form an integral part of the personalised care team to provide cancer patients with information and support that meets their individual needs.As part of a team, the cancer support worker will speak to patients to discuss what matters to them through a holistic conversation and assessment. The cancer support worker will provide cancer information and/or signpost them to support services and resources and will develop a personalised care plan which is shared with the patient and specialist team and that this and other communications are documented on the Somerset Cancer Registry.The post holder will work closely with the Cancer Information and Support Service team, covering the patient phone line and will contribute to the organisation and delivery of health and well-being events.
