Job summary
As thePersonalised Care Team Lead, you will oversee and support the SocialPrescribing Link Workers within Rockingham Forest PCN. This role is aboutempowering patients to take control of their health and wellbeing. Our LinkWorkers help people with a wide range of needs whether they are managinglong-term conditions, experiencing low mood or loneliness, or dealing withcomplex social challenges.Please note this is a part time role offering 30 hours per week

Main duties of the job
You'll lead a dedicated team of Social Prescribing Link Workers, Health & Wellbeing Coaches, and Care Coordinators, providing day-to-day supervision, guidance, and opportunities for staff development.You will be working closelywith GP practices, community partners, and voluntary organisations, you'll helppatients access the right support at the right time whether that's connectingwith local services, building confidence to manage their health, or

Job responsibilities
Primary Care Networks (PCN) are a group of local General Practicesurgeries working in collaboration with one another to provide services to itslocal communities and patient groups. Lakeside Healthcare Group is responsiblefor 4 PCNs across 3 Localities and providing services to approximately 200,000patients across these networks.The Personalised Care roles (Social Prescribing Link Workers, Health& Wellbeing Coaches and Care Coordinators) play an important part inproviding extra time, capacity, and expertise to support patients to beactively involved in managing their health and to make choices that are rightfor them. Working to the What Matters To Me model, personalised care deliveredis focused on the strengths and needs of the individual.The successfulcandidate will be responsible for leading and providing the day-to-daysupervision and support for Personalised Care Team within the PCN; evolving anddeveloping business as usual processes and working with the Digital &Transformation Manager to develop new initiatives to optimise the care optionsavailable for the PCN patients and communities. A first line of contact andguidance for site leads on the introduction and induction of new personalisedcare roles into the PCN.
