If you areenthusiastic and have great people and communication skills, we would like tohear from you.The role is a split role within the surgery consisting of HCA clinics and being part of our Frailty Team which will including calling patients and visiting them in their homes.

Main duties of the job
The HCA role involves a variety of duties includingPhlebotomy (venepuncture), ECG's, BPs, and urinalysis, long-term condition checks, Health checks and wound checks. (Thislist is not exhaustive and may change to the needs of the nursing team).The fundamental part of the HCA Frailty role is to meet the primary care needs of an identified group of peopleidentified as frail and needing further support in the community. You would be expected to undertake home visits in accordance with the relevant protocols, support patients and families to help patients safely stay at home.
