Job summary
We are seeking an
Experience
d and motivated Complex Discharge Coordinator to join our dynamic team on a 6-month secondment. This role is pivotal in supporting safe, timely, and person-centred discharges for patients with complex needs across our hospital sites.The post holder will be based primarily at Heartlands Hospital, with regular cross-site working at Solihull Hospital, and occasional support required at Queen Elizabeth and Good Hope Hospitals to ensure continuity of service.As part of a 7-day service, you will work a flexible rota including weekends and bank holidays, ensuring effective discharge planning and coordination between clinical teams, patients, families, and community partners.7 day working including weekends and bank holidays - Service cover from 8am to 6pm
Main duties of the job
The post holder will have dedicated wards and be responsible for the tracking and progressing of individual delayed discharges in specific areas, whether Health or Social care responsibility. This includes maintaining accurate computerised and manual records, inputting data onto the Trust's computer systems, responding to and producing information requests and ensuring that all patient records are kept up to date with any

relevant to the patient's discharge pathway.The post holder is the first point of contact within the Complex Discharge Hub and in conjunction with the wider Complex Discharge Hub Team, actively contributes towards the achievement of department and directorate KPI's (Key Performance Indicators).The post holder will assist the Team Leader, Group Manager, Operations Manager and Clinical Leads of the Hubs in gathering, collating and analysing data. This may include issuing notices in line with legislationThe post holder is responsible for liaising and communicating with other members of the Complex Discharge Hub to ensure they receive regular updates throughout the dayThe post holder will be required to support the escalation arrangements for delayed patients. Deputise as appropriate for the Team Leader, and contribute to the escalation meetings. Work will include providing progress reports and data, using Department/Trust systems and relaying feedback for members not in attendance.
