This Band 4 Complex Discharge Coordinator role is a six‑month secondment based mainly at Heartlands Hospital with regular cross‑site work at Solihull and occasional support at Queen Elizabeth and Good Hope Hospitals. Working as part of a 7‑day service (rota covering 8am–6pm, including weekends and bank holidays), you will coordinate safe, timely and person‑centred discharges for patients with complex needs. Day‑to‑day duties include tracking and progressing delayed discharges, maintaining and updating Trust records and IT systems, producing data and reports, liaising with clinical teams, patients, families and community partners, supporting escalation processes and deputising for the Team Leader when required. You will work within the Complex Discharge Hub to help meet department KPIs and may issue notices in line with legislation where necessary.

This opportunity will suit organised, compassionate carers or administrators with strong IT skills and experience of patient administration systems; essential qualifications include good general education plus ECDL or NVQ level 3 in Business Administration/Customer Care (or equivalent). You’ll need advanced ability in Excel, Word and PowerPoint, a sound knowledge of confidentiality and the Data Protection Act (2018), and the confidence to prioritise without direct supervision. Previous health or social care administration, multidisciplinary team experience or supervisory experience are desirable. The post is Band 4 (salary £27,485–£30,162) on a fixed‑term contract and will require a DBS check.
