Support continuity of care across Surrey Heath as the DES and Projects Care Coordinator for Surrey Heath Community Providers Ltd, based in Camberley (GU16). You’ll coordinate Direct Enhanced Services (DES) and core PCN projects — maintaining EMIS clinics and user access, running reports, managing shared inboxes, organising clinics and Enhanced Access sessions, and helping with audits, quality returns and SOP updates. Day-to-day work is a mix of admin, digital systems and patient-focused signposting, working closely with clinicians and admin teams to keep services running smoothly for over 100,000 local patients.
This permanent role (full-time; salary £27,456 a year, pro rata for part time) is ideal for organised, compassionate carers and administrators with NHS or social care experience, strong IT skills and an eye for data quality. You’ll get to shape PCN projects such as Dermatology, SDUC and Heart Health Checks, learn and improve processes with TeamNet and EMIS, and travel locally between practices.
