Job summary
The Projects and DES Care Co-ordinator will provide high-quality administrative and coordination support to ensure seamless delivery of PCN services, with a focus on Direct Enhanced Services (DES) implementation, patient care continuity, data quality and service improvement.

Main duties of the job
We arelooking for a motivated andwell-organised Projects and DES Care Co-ordinator to join Surrey Heath PrimaryCare Network (PCN). This varied role supports the coordinationand delivery of Direct Enhanced Services (DES) and key PCN projects, ensuringhigh-quality, patient-centred care across our practices.You will work closely withclinical and administrative teams to help patients navigate health and careservices, supporting continuity of care and smooth communication across thenetwork. The role includes maintaining EMIS clinics and user access, runningreports, managing shared inboxes, coordinating clinics and Enhanced Accesssessions, and ensuring data quality and compliance.You will also assist withaudits, quality returns, and PCN projects such as Dermatology, SDUC, and HeartHealth Checks, updating SOPs and TeamNet content as processes evolve.This is an excellentopportunity for someone with strong administrative, organisational, and digitalskills who enjoys teamwork and improving patient
