Community Health and Social Care Assistant at Derbyshire Community Health Services NHSFT, based at Perth House Assessment Hub in Chaddesden, Derby (DE21). This hands‑on role supports people recently discharged from hospital or needing urgent community care, helping them regain independence through practical support, personal care and agreed treatment programmes. You’ll work as part of a close‑knit multi‑disciplinary team, carrying out individual care with professional autonomy, maintaining accurate records and supporting clinicians to deliver high standards of patient care. The post is permanent, part‑time on a 4‑on, 4‑off rota (mornings 07:15–14:45 and evenings 14:30–22:00), includes weekends and bank holidays, sits at Agenda for Change Band 3 with a pro‑rata salary of £24,937–£26,598 and is subject to a DBS check.

This role is ideal for people with previous experience in health or social care who can provide personal care, have GCSEs in English and Maths (or equivalent) or NVQ3, basic IT skills, good communication and a willingness to travel within a locality.
