As a Care Co-ordinator with Inclusion Healthcare CIC based at Charles Berry House in Leicester (LE1), you will work part‑time (18.75 hours/week, permanent) coordinating person‑centred care for people experiencing homelessness and complex social needs. The role is hands‑on and community‑facing: liaise with GPs, nurses, social care and voluntary services; arrange and support appointments; undertake home and outreach visits; maintain accurate SystmOne records; help people navigate barriers such as language, literacy or digital exclusion; arrange interpreters or escorts where needed; and take part in multidisciplinary team meetings. You’ll use Office 365, AccuRx and other digital tools, work with trauma‑informed practice, and be flexible to provide cover across Inclusion Healthcare sites when required. Pay is £13,250 actual (FTE £26,529) and the post is subject to a satisfactory DBS check and right to work checks.

This role will suit compassionate, organised carers and administrators—particularly women seeking meaningful, flexible hours—who have experience coordinating care or supporting the public, strong communication and IT skills, and GCSEs (or equivalent) in English and Maths.
