This Community Healthcare Assistant role with Norfolk Community Health and Care NHS Trust places you at the heart of community nursing in Central Norwich (Norwich Community Hospital, NR2). Working within a multidisciplinary team of community nurses, assistant practitioners and phlebotomists, you will visit delegated patients in their own homes and work across clinic settings to follow prescribed care plans aimed at preventing hospital admission. Day-to-day duties include planning and prioritising visits, completing patient-held records and daily SystmOne entries, basic clinical observations and treatments such as wound care, blood glucose checks and administration of medicines (for example insulin), health promotion and clear communication with patients and carers. You will also share indirect tasks such as answering calls and arranging appointments, maintain equipment safely and adhere to infection-control policies; some roles involve exposure to bodily fluids and require safe handling practices.

This post is suited to caring, organised people with experience in a health-care setting (NVQ2 or equivalent and GCSEs in English and Maths are essential), who are flexible, good at communicating and able to work both independently and as part of a team.
