Modern virtual ward role using digital monitoring and supportive multidisciplinary teams.
Work as a Virtual Ward Health Care Support Worker with Midlands Partnership NHS Foundation Trust, delivering acute care in patients' homes across Lichfield, Burntwood, Tamworth and Burton (B78). This Band 3, permanent full‑time role supports a new community virtual ward for frailty and sub‑acute patients, helping avoid hospital admissions and reduce length of stay. You will visit patients, record observations, support remote monitoring via digital dashboards, perform delegated clinical duties (eg. blood pressure, blood glucose), provide patient education and liaise with multidisciplinary teams. The role requires a valid, in‑date driving licence, access to a car, a DBS check, and vocational level 3 (or working towards). Please note this post is not eligible for visa sponsorship and interviews are face to face.
