An experienced PCN Care Co‑ordinator is needed by Morley and District PCN to shape personalised primary care across Morley (Shenstone House, LS27 7PX) and Tingley/Leigh View (WF3 1RQ). This part‑time, permanent role (salary £25,800–£26,800 pro rata) is ideally suited to someone with a background in care coordination, adult health and social care or strong administration skills who enjoys working directly with people, families and carers. You’ll manage a caseload in the community and practice, create and review personalised care and support plans, book and follow up appointments across primary, secondary and community services, and act as a single point of contact to help patients—particularly frail, elderly and people with long‑term conditions—navigate joined‑up care. Accurate record keeping using GP systems (System1, SNOMED coding) and adherence to safeguarding and information governance are essential.
