Step into a hands-on community role as a Rapid Response Assistant Practitioner (Band 4) with NELFT in Leytonstone (E11). You will join a forward-thinking multidisciplinary Rapid Response team — working alongside nurses, physiotherapists, occupational therapists, social workers and community psychiatric nurses — delivering urgent MDT care within 0–2 hours to prevent avoidable hospital admissions. The job involves clinical visits to people’s homes, care and residential settings, responding to falls and other urgent needs under the direction of senior clinicians, and using a mix of nursing and therapy skills in the patient’s environment. This permanent, full-time post demands flexible shift patterns across seven days and 24-hour cover; applicants should be prepared to use their own car (mileage and parking are claimable). The role suits Band 4 HCAs, hybrid workers, assistant practitioners or nurse associates with at least two years’ relevant NHS or therapy support experience, good IT and communication skills, and the ability to work calmly under pressure in a close-knit team.
