Work as a Community Adolescent Senior Support Worker with Hampshire and Isle of Wight Healthcare NHS Foundation Trust, based at Adelaide Health Centre, Southampton (SO16). You’ll join the Closer2Home Team delivering intensive, time‑limited home treatment as an alternative to inpatient admission for young people whose needs would otherwise require hospital care. The role is hands‑on and clinical, providing direct and indirect care under delegated supervision as part of a multi‑disciplinary team; you will co‑ordinate with CAMHS, children’s services, bed and case managers and local inpatient units to support admissions avoidance and safe, supported discharges. This post is open only to applicants employed within the Hampshire & Isle of Wight NHS system (including Hampshire IOW Healthcare NHS Trust, Isle of Wight NHS Trust, University Hospital Southampton, Portsmouth Hospitals, South Central Ambulance Service, Hampshire Hospitals Foundation Trust, or the Hampshire & Isle of Wight ICB). A DBS check is required.

This Band 4, full‑time role (£27,485–£30,162 pro rata) is a 12‑month secondment with flexible working options and strong emphasis on training, development and the Trust’s CARE values of compassion, accountability, respect and excellence. It suits experienced carers and compassionate women seeking a practical, values‑led NHS role: you’ll need NVQ Level 3 (or equivalent experience), Maths/English Level 2 (GCSE C/4 or above) and a Care Certificate (or to complete within 12 weeks).
