Open only to applicants from Hampshire & Isle of Wight NHS system, which is limited to;Hampshire IOW Healthcare NHS Trust, Hampshire and Isle of Wight ICB, Portsmouth Hospitals University NHS Trust, Isle of Wight NHS Trust, University Hospital Southampton Foundation Trust, South Central Ambulance Service or Hampshire Hospitals Foundation Trust.This is an exciting opportunity to join the Closer2Home Team, which is a intensive home treatment service that supports young people and families in the community as an alternative to an inpatient admission.The service operates across Wessex and Dorset, working in partnership with the Wessex and Dorset Tier 4 Provider Collaborative (W&D PC) to deliver a clinical model of evidence-based intensive home treatment as a safe and effective alternative to hospital admission.As a Community Adolescent Senior Support Worker (CASS) you will work as a member of a multi-disciplinary team which provides an alternative to Tier 4 admission and facilitates supported discharges from hospital. With colleagues you will be responsible for delivering time-limited programmes of intensive home treatment to young people who would otherwise have been admitted.
Main duties of the job
With team colleagues you will be responsible for delivering time-limited programmes of intensive home treatment to young people who would otherwise have been admitted. Young people requiring home treatment will have a range of needs commensurate with requiring inpatient services, but be offered intensive home treatment as an alternative.Young people requiring home treatment will have a range of needs commensurate with requiring inpatient services but be offered intensive home treatment as an alternative. The team work collaboratively with community CAMHS, children's services, colleagues involved in 'transforming care,' bed and case managers and local inpatient units; with inter-agency networking embedded in the service model.The team will work collaboratively with community CAMHS, children's services, colleagues involved in 'transforming care,' bed and case managers and local inpatient units; with inter-agency networking embedded in the service model.The post holder with have specific responsibilities to undertake a range of delegated clinical duties in a community mental health setting. The post holder will contribute to the growth and maintenance of key functions of the clinical area / Team by carrying out assigned tasks, involving direct and indirect care to patients under minimal supervision and as directed by the qualified staff and / or Team Managers.
