Job summary
The Integrated Care Partnership is a large GP practice and a standalone Primary Care Network (PCN). With a Patient population of 32,000, the Practice is ideally placed to benefit from the unique position of being a single Practice PCN.We are looking for someone to help provide anefficient service to patients presenting to, electronically requesting ortelephoning the practice
Main duties of the job
To assist the doctors, other clinicians and management in the provision of a high quality serviceReceive and direct patients on arrival in a courteous and efficient mannerProcessand effectively signpost patients to the appropriate healthcare professional,depending on the presenting conditionAnswering general enquiries, explaining processes and responding to requestsAnswering telephone calls, transferring calls or dealing with the callers' requests appropriatelyProcess patient requests for appointmentsReceiving and recording requests for home visits and messages for the doctors & nursesProcess repeat prescription requests in accordance with company proceduresGiving results to patientsEnter patient data onto the clinical system (EMIS) and code appropriatelyInitiate contact with and respond to requests from patients, team members and external agenciesTo provide cover for colleagues absences

To assist the doctors and other clinicians in the provision of a high quality serviceReceiving and directing patients on arrival in a courteous and efficient mannerAnswering general enquiries, explaining processes, responding to electronic requestsAnswering telephone calls coming into the surgery and dealing with callers as appropriateReceiving and recording requests for home visits and messages for the doctors & nursesSeeking patient information and results from hospitals and other organisationsGiving test results information to patientsBook transport for patients having to attend hospital appointments.Initiating computerised prescriptions as requested by patients.Registering new patients onto the clinical system (both permanent & temporary patients)Entering patient data onto the clinical systemContacting/chasing patients as requested by the Clinicians.Booking of appointmentsPhotocopyingEmailing outside agenciesTo work as a positive and active member within the Practice Team.You will be expected to take part in an annual appraisal at which your performance will be reviewed and your training needs discussed.Attend relevant meetings as required including staff meetings.
