Open to internal MPFT employees onlyWe are offering an exciting and unique new role as a Band 4 Health Care Support Worker within our Integrated Mental Health Teams (IMHT) across South Staffordshire, supporting our new First Transitions Pathway.The First Transitions Pathway is a new service to help support individuals with complex mental health as they move from the structured support of forensic services to the community-focused care offered by our Integrated Mental Health Teams. This transformation is underpinned by a central commitment to promoting sustained recovery, fostering meaningful integration within the community, and empowering service users to lead fulfilling lives. This specialised team will work in close collaboration and partnership with the six IMHTs spanning South Staffordshire.
Main duties of the job
You will be instrumental in providing practical and emotional support directly to service users, both during the immediate transition phase and in the crucial weeks and months that follow. Their focus will be on building rapport, implementing effective engagement strategies, and facilitating access to community resources.As part of this role you will be expected to:Provide intensive and assertive outreach to service users at risk of disengagement, aiming to prevent relapse.Provide practical support within community settings,

ing immediate needs and implementing engagement strategies.Be curious and motivated to make a difference to this cohort of individuals.
