A Band 4 Health Care Support Worker role within MPFT’s First Transitions Pathway, based in Lichfield (WS13) and working across Integrated Mental Health Teams in South Staffordshire. This internal MPFT-only post supports people moving from forensic services into community care, offering practical and emotional support during the immediate transition and in the crucial weeks and months that follow. You will deliver intensive, assertive outreach to those at risk of disengagement, build rapport, use engagement strategies, and help people access community resources to reduce relapse and promote sustained recovery. The role is permanent, full-time, Agenda for Change Band 4 (salary £27,485–£30,162 pro rata) and requires DBS clearance.

This opportunity is well suited to experienced carers and Health Care Support Workers with an NVQ3, Foundation degree or equivalent and proven work with adults with mental health needs, ideally within multi‑agency or forensic settings.
